Request Statutory Deposit Confirmations or Certifications
Requests for statutory deposit confirmations and/or certifications must be submitted to the Department in writing.

Requests should not be sent to the Department’s custodian bank: U.S. Bank.
Requests must include the following:
- The Company Name, NAIC CoCode, and the US Bank Account Number.
- The period ending for the request (i.e., March 31, 2022).
- An email address, a return mailing address, or a return envelope so that the statutory deposit confirmation and/or certification can be provided to the correct address and recipient.
The Department charges $25.00 for each statutory deposit confirmation and/or certification. A check made payable to the North Carolina Department of Insurance must accompany all written requests. At this time, electronic payment is not available for these requests. Requests will not be processed until payment is received.
