Request Statutory Deposit Confirmations or Certifications

Requests for statutory deposit confirmations and/or certifications must be submitted to the Department in writing and must include in one submission:

A completed Certificate of Deposit Request Form
Pursuant to N.C.G.S. 58-6-5(1) a $25.00 check, per account, made payable to the North Carolina Department of Insurance

Requests submitted without payment will not be processed.

At this time, electronic payment is not available for these requests.
